The following groups have been approved to receive a UND Affiliate U Card. An ID number (EMPL #) must be issued prior to receiving your U Card. To request your ID number, please print, complete and submit the form below to the Associate Vice President for Finance and Operations, Box 8364. There is a $20 fee for each card which may be paid by cash, check or credit card. If a department chooses to pay for the card, please provide a department memo indicating the account number to be charged.

Once this form has been approved and signed, you must bring the form to the U Card Office in order to get your U Card. At that time, you will also need to provide a government issued, photo ID (driver's license, passport, state ID card).
